About this course

This training focuses on clinical documentation within the EMDR 8-Phase Protocol, helping therapists develop clear, structured, and ethically sound documentation that reflects the progression of EMDR therapy. Participants will learn how to document treatment planning, target development, processing progress, and clinical decision-making across the phases of EMDR therapy. The training reviews key elements clinicians should capture in their notes to demonstrate adherence to the EMDR model while accurately reflecting client responses, therapeutic interventions, and treatment outcomes. The session provides guidance on documenting clinical work within each phase of the EMDR process, including history taking, preparation, assessment, desensitization, installation, body scan, closure, and reevaluation. Emphasis is placed on creating documentation that supports clinical clarity, continuity of care, and ethical record keeping. Through structured examples and applied discussion, clinicians will gain practical strategies for documenting EMDR sessions in ways that are efficient, clinically meaningful, and aligned with trauma-informed practice. This training is included for members of the Kathy on the Couch Membership and is also available as a standalone training for individual purchase.

Learning Objectives

  • • Write SMART treatment plan goals for EMDR therapy that include a specific target, measurable SUD/VOC indicators, and a defined time frame
  • • Distinguish between progress notes and psychotherapy notes, including their respective HIPAA protections and documentation requirements
  • • Document all 8 phases of the EMDR protocol using phase-specific clinical language
  • • Identify required documentation elements for incomplete EMDR sessions, including containment strategy, distress at close, and safety check
  • • Apply the Golden Thread framework to create a consistent, auditable clinical record from assessment through outcomes

  • Kathy Couch

    Kathy founded Rewired360, a EMDR Consulting/Training business and Willow Creek Counseling, an EMDR therapy business. Certifications are held with EMDRIA as a EMDR Certified Therapist/Approved Consultant, and a Fellow in Thanatology with the Association of Death Education and Counseling, and is a Level 2 trained Gottman Institute clinician.

    Kathy has 10 years of teaching experience at Boise State University, with specializations in in program evaluation, research and small business development. Specialties include areas of death, grief, bereavement, couples, and has experience with addiction, child welfare, probation and parole, TF-CBT, substance abuse and domestic violence. She regularly provides EMDR and Couples Intensives and lives in Twin Falls, Idaho with her family daughter, twin boys and family dogs.
